Certificate of Dental Screening – What Do You Need to Know?
Summer is winding down and that means it is almost time to send your kids back to school. Along with school supply shopping and haircuts, getting all those forms filled out for enrollment is probably on your to-do list. One of these required forms is called a certificate of dental screening. Here you can find more information about this certificate and what you as a parent need to do to make sure your child can successfully enroll in school.
What is a Certificate of Dental Screening?
It is an Iowa mandated health screening that proves a student has had an oral examination completed by a certified dentist, dental hygienist, physician, nurse, or other qualified professional prior to school enrollment.
Which Children Are Required to Have a Certificate of Dental Screening?
Students enrolling in kindergarten through ninth grade are required to have this certificate. A screening for children in kindergarten is valid from age 3 to four months after the enrollment date. A screening for children in 9th grade is valid one year prior to four months after the enrollment date.
Do All Des Moines Area School Districts Require Dental Screenings?
Yes, they do. An Iowa law that effectively began in 2008 requires that every child (K-9) enrolling in an Iowa elementary or high school is required to have a dental screening. This law is to ensure that the oral health of students is monitored and improved.
How to Schedule an Appointment
A certificate of dental screening can be completed by a licensed dentist, dental hygienist, physician, nurse, or physician’s assistant. If your child is in kindergarten, they can get their dental screening completed by the school nurse. If you are in central Iowa, Pediatric Dentistry of Central Iowa has offices in both Ankeny and Des Moines and would be more than happy to get an appointment scheduled for your child.
Submitting Dental Screening to the School Nurse
Submitting the certificate of dental screening is easy. Just print off the form, make sure it is filled out by the person completing the screening, and return it to the school your child will be attending.
